We are seeking a compassionate and dedicated CHHA to join our team. The ideal candidate will provide essential support to patients in their homes or inpatient settings, ensuring comfort and dignity during their end-of-life journey. The CHHA will work closely with healthcare professionals to deliver high-quality patient care, assist with daily living activities, and maintain accurate medical documentation.

Duties

• Provide personal care services to patients, including bathing, grooming, and dressing.
• Assist with mobility and transfers to ensure patient safety and comfort.
• Support patients with meal preparation and feeding as needed.
• Monitor and report changes in patients' conditions to the healthcare team.
• Maintain a clean and safe environment for patients by performing light housekeeping tasks.
• Document patient care activities accurately in compliance with medical documentation standards.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Provide emotional support and companionship to patients and their families during this challenging time.

Requirements

• Previous experience in home community care or inpatient settings is preferred.
• Knowledge of anatomy and basic medical terminology is beneficial.
• Familiarity with caregiving practices and patient care protocols is essential.
• Experience in a medical office or clinic environment is a plus.
• Ability to perform phlebotomy tasks if required by the patient's care plan.
• Strong communication skills and the ability to work effectively within a team.
• Compassionate demeanor with a commitment to providing high-quality patient care.
• Must be able to pass background checks as required by healthcare regulations.
