# Per Diem Telehealth Facilitator – Magic Valley
📍 Location: Magic Valley, Idaho 🕒 Schedule: Per Diem / As Needed — Approximately 10–25 hours per week depending on case volume and facility needs 💰 Pay: 20–22/hour depending on experience, with opportunity for additional attendance-based compensation

EverCare Mobile Health is seeking a Per Diem Telehealth Facilitator to support patients and providers throughout the Magic Valley area. This position works directly inside long-term care and residential facilities, helping patients connect with EverCare mental health providers through telehealth.
This is an as-needed position, with hours expected to average approximately 10–25 hours per week, depending on patient volume, provider schedules, and facility needs. Hours are not guaranteed and may fluctuate from week to week.

What You'll Do
As a Telehealth Facilitator, you will serve as the on-site connection between patients, facility staff, and EverCare's remote healthcare providers.
Responsibilities include:
- Travel to assigned long-term care and residential facilities throughout the Magic Valley area
- Set up and facilitate telehealth appointments between patients and EverCare providers
- Assist patients with navigating the telehealth process and technology
- Help obtain and coordinate required patient consents and documentation
- Communicate scheduling concerns, patient needs, and changes to the appropriate EverCare team members
- Collaborate professionally with facility staff and interdisciplinary care teams
- Maintain patient privacy and HIPAA compliance at all times
- Follow EverCare and facility safety, infection-control, and emergency procedures
- Respond appropriately when patient safety or other concerns arise
- Maintain reliable communication with EverCare during scheduled working hours
- Attend required meetings, training, or supervision as applicable
- Provide consistent, professional, and compassionate interactions with patients and facility partners
What We're Looking For
Required
- High school diploma or equivalent
- Valid driver's license
- Reliable personal transportation
- Current automobile insurance
- Ability and willingness to travel between facilities throughout the Magic Valley area
- Ability to pass required Enhanced IDHW Idaho background screening
- Dependable attendance and ability to manage a flexible, case-volume-driven schedule
- Comfortable using computers, tablets, video conferencing, and other technology
- Professional communication skills
- Comfortable working independently in assisted living, memory care, skilled nursing, and other residential care environments
- Ability to maintain confidentiality and comply with HIPAA requirements
Preferred
- CPR certification
- Previous healthcare, behavioral health, caregiving, medical assistant, social services, or senior-care experience
- Experience working in long-term care or residential facilities
- Familiarity with electronic health record (EHR) systems
- Experience assisting patients with telehealth or other healthcare technology
