Clinical Nurse Specialist (CNS) - Behavioral Health — Toledo, OH

Key Responsibilities

• Develop, coordinate, and update individualized behavioral health treatment plans based on patient needs and progress.
• Conduct assessments, evaluations, admissions coordination, and crisis intervention within scope of practice.
• Coordinate referrals and collaborate with interdisciplinary teams, internal departments, community agencies, and external partners to support continuity of care.
• Evaluate treatment effectiveness, modify interventions, and maintain accurate documentation and reporting.
• Participate in treatment team meetings, training, and program improvement activities while supporting lower-level providers when appropriate.

Requirements

• Master's degree.
• Current BLS from the American Heart Association or American Red Cross.
• Current active Ohio Registered Nurse (RN) license with Clinical Nurse Specialist (CNS) and a Mental Health or Psychiatry specialty, maintained in accordance with applicable state and certification requirements.
• Ability to work with patients who may have behavioral or psychiatric conditions and meet role-related physical requirements, including occasional lifting up to 60+ lbs.

• NP – Nurse Practitioner with Mental Health or Psychiatry Specialty
