A Day After Day Home Care Agency is seeking an outgoing, professional, and relationship-focused Community Liaison to help build referral partnerships and expand our community presence.

Part-Time: Approximately 10–15 hours/week

Location: Indianapolis & surrounding areas

Schedule: Variable based on business needs

Mileage: Approved business-related mileage reimbursed according to company policy

Growth: Opportunity for increased hours and responsibilities as the company grows

Responsibilities

• Build relationships with hospitals, nursing/rehab facilities, senior living communities, hospice, and healthcare professionals.
• Conduct community visits, networking, and outreach.
• Promote our home care and transportation services.
• Follow up with referral partners and develop new referral opportunities.
• Track outreach activities and referrals.

Qualifications

• Strong communication and networking skills
• Outgoing, professional, and self-motivated
• Comfortable with phone calls and face-to-face meetings
• Healthcare, home care, sales, or community outreach experience preferred
