Able Home Healthcare is looking for a compassionate and dedicated Home Health Physical Therapist Assistant to join our growing team in San Diego, CA. Responsibilities
- Implement physical therapy treatment plans established by the supervising Physical Therapist
- Provide in-home therapeutic exercises, gait training, and functional mobility interventions
- Monitor and document patient progress, reporting changes in condition to the supervising PT
- Educate patients and family members on home exercise programs and safe mobility techniques
- Coordinate care with the interdisciplinary home health team to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with Medicare and agency standards
- Travel to patient homes throughout the San Diego area on a scheduled caseload
Requirements
- Associate's or Bachelor's degree from an accredited Physical Therapist Assistant program
- Active California PTA license in good standing
- Previous home health or rehabilitation experience preferred
- Strong knowledge of Medicare home health guidelines and documentation requirements
- Excellent communication, time management, and organizational skills
- Reliable transportation, valid driver's license, and auto insurance
- Compassionate, self-motivated, and able to work independently in a field-based setting
