Full job description
Key Responsibilities
- Develop, coordinate, and update individualized behavioral health treatment plans based on patient needs and progress.
- Conduct assessments, evaluations, admissions coordination, and crisis intervention within scope of practice.
- Coordinate referrals and collaborate with interdisciplinary teams, internal departments, community agencies, and external partners to support continuity of care.
- Evaluate treatment effectiveness, modify interventions, and maintain accurate documentation and reporting.
- Participate in treatment team meetings, training, and program improvement activities while supporting lower-level providers when appropriate.
Requirements
- Master’s degree.
- Current BLS from the American Heart Association or American Red Cross.
- Current active Ohio Licensed Independent Chemical Dependency Counselor (LICDC) license required and maintained in accordance with state continuing education requirements.
- Ability to work with patients who may have behavioral or psychiatric conditions and meet role-related physical requirements, including occasional lifting up to 60+ lbs.
- LISW – Licensed Independent Social Worker
- LPCC – Licensed Professional Clinical Counselor
- IMFT – Independent Marriage & Family Therapist
- Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP) with a Mental Health or Psychiatry specialty
