About Us
The LPN Care Coordinator Team Lead is a senior, field-based role within Ennoble Care’s Care Coordination program. This position combines hands-on, embedded care coordination responsibilities with the oversight, leadership, and operational management of a team of 8–12 Embedded Care Coordinators (CCs) across DaVita clinics.
The Team Lead serves as the subject matter expert for facility-based care coordination, acting as a trusted bridge between Ennoble Care’s clinical teams, DaVita clinical staff, and the broader care coordination function.

KEY RESPONSIBILITIES
Clinical Operations & Patient Care
- Maintain consistent embedded presence at assigned clinics and serve as a practicing CC for Ennoble Care's patient panel where applicable (depending on panel size expectations above)
- Execute all core daily CC responsibilities such as prescription management, lab/radiology orders, referral coordination, pre-visit check-ins, missed treatment outreach, and post-discharge follow-up
- Function as the “glue” between facility staff and Ennoble Care’s services, including house calls, hospice, and ancillary programs
- Conduct proactive rounds with both patients and DaVita clinic staff, respond to acute patient needs, and escalate to providers as appropriate
- Meet the same daily task minute goal as a standard Team Lead (400 mins/day), unless engaged in approved training or onboarding activities
- Support facilitated telehealth and RPM programs within assigned facilities
Team Leadership & Oversight
- Directly oversee a team of 8–12 Embedded CCs across multiple facilities and/or markets
- Provide day-to-day guidance, coaching, and support — ensuring all CCs execute daily responsibilities consistently and to standard
- Serve as the primary point of escalation and problem-solving resource for the embedded CC team
- Monitor individual and team performance against KPIs; address performance gaps directly or escalate as needed
- Coordinate with the providers to ensure they have everything needed from the CC team
- Partner with leadership on staffing decisions, performance management, and team development
- Set and model the standard for documentation quality, reliability, communication, and patient engagement
- Backfill for Embedded CCs as needed to maintain care continuity
- Travel to facilities to oversee Care Coordinators and assist as needed
Training & Onboarding
- Support onboarding and training of new Embedded CCs, ensuring completion of all required credentialing and training modules for both Ennoble and DaVita
- Support new Embedded CCs through their ramp period, setting performance expectations and conducting regular check-ins
Clinic Relationships & Collaboration
- Build and maintain strong working relationships with DaVita clinic staff to ensure smooth day-to-day coordination
- Represent the CC team professionally in any joint meetings with DaVita
Performance Tracking & Process Improvement
- Monitor and report on clinical and operational metrics weekly; track and escalate any metric trending off target in a timely manner
- Ensure accurate, up-to-date patient documentation in OA at all times
- Flag operational issues, workflow gaps, or patient concerns promptly and propose solutions
- Help refine and update CC-facing tools, checklists, and SOPs as the program evolves
QUALIFICATIONS
Required
- CMA or LPN license required
- Valid Driver's license and access to a vehicle required
- At least 2 years of management experience in a healthcare setting
- Knowledge of principles and processes for providing customer and patient services, including customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction
- Knowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources
- Proficiency in the structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammar
- Understanding of principles and methods for curriculum and training design, teaching and instruction for individuals and groups, and the measurement of training effects
- Familiarity with principles and procedures for personnel management
Preferred
- Experience in a lead, senior, or supervisory care coordination role
- Familiarity with facility-based operations and workflows
- Experience onboarding or training colleagues in care coordination workflows
COMPENSATION & BENEFITS
- Starting salary: $50,000
