About the Role
HOMECARE RESCUE LLC is looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing Home Health team serving Pasco County. Candidate will initiall work on a PRN basis. Responsibilities
- Provide skilled nursing care to home health patients throughout Pasco County in accordance with physician-approved care plans
- Perform clinical assessments, wound care, medication administration, and IV therapy as ordered
- Monitor and document patient progress, vital signs, and changes in condition
- Educate patients and family members on disease management, medications, and home care techniques
- Collaborate with RNs, therapists, and the interdisciplinary care team to ensure continuity of care
- Maintain accurate and timely clinical documentation in compliance with state and federal home health regulations
- Communicate patient status updates and concerns to supervising RN or physician promptly
Requirements
- Active Florida LPN license in good standing
- Familiarity with WellSk
- Previous home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of wound care, medication management, and patient assessment
- Reliable transportation and valid driver's license for travel throughout Pasco County
- Excellent communication, time management, and organizational skills
- Ability to work independently with minimal supervision in a home care setting
- CPR/BLS certification required
