Job Description Summary

$28.00 an hour.

The Weld County Department of Public Health and Environment is seeking 2 part-time (28 hours per week) Community Health Workers to provide health education, mental health resources and social services to members in the community who have limited access to medical care. In this role, you communicate with underserved people, particularly women, children, and migrant workers, in order to determine the care, they need and connect them to the right services. Our ideal candidate has at least an associate degree in health education or a related field and is bilingual or multilingual in languages that our community members speak.--

Job Description

Increase Community Health Awareness - 25%

  • Educate community members about preventive healthcare measures, disease management, and healthy lifestyle choices.
  • Empower community members to take ownership of their health and advocate for positive changes in their neighborhoods, schools, and workplaces.

Improve healthcare access. - 25%

  • Facilitate access to healthcare services, including vaccinations, screenings, and treatments, particularly for underserved populations.

Enhance health literacy. - 20%

  • Help community members understand medical information, navigate health care systems, and advocate for their own health needs.
  • Pursue ongoing training and professional development opportunities to stay abreast of best practices, emerging health issues, and new technologies in community health work.

Promote behavior change. - 20%

  • Encourage individuals and families to adopt healthy behaviors such as exercising regularly, eating nutritious foods, and seeking medical care when necessary.
  • Respect and incorporate cultural beliefs, values, and practices into health education and interventions to ensure they are relevant and effective for diverse communities.
  • Offer tailored assistance to individuals facing socioeconomic challenges, language barriers, or other barriers to health access.

Strengthen community partnerships. - 10%

  • Collaborate with local health care providers, government agencies, nonprofits, and other stakeholders to address community health needs effectively.

Required Qualifications

Required Education

  • High School Diploma/GED or 2 years job mentoring or on the job training .
  • Associate's Degree degree in health related field.
  • An equivalent combination of education and/or experience may be considered.

Experience Qualifications

  • 1 year understanding of local social and health resources to make appropriate referrals.

Preferred Education

  • Associate's Degree degree in a health related field.

Skills and Abilities

  • Understanding of local social and health resources to make appropriate referrals. (High proficiency)
  • Ability to manage data. (Medium proficiency)
  • Comfortable with public speaking and leading group discussions and workshops. (High proficiency)
  • Strong organization and case management skills (High proficiency)
  • Bilingual in Spanish and /or other languages spoken in the community (High proficiency)
  • Candidate must pass criminal background check prior to employment start date.

Licenses and Certifications

  • Community Health Education Specialist preferred.
  • Pre-employment substance screening is required for this position.
  • This position is subject to a pre-employment drug screening. Employment is contingent upon successfully passing the screening in accordance with Weld County policy.

This position is non-exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act and is therefore eligible for overtime pay.
