Full job description
Clinical Oversight & Competency Validation
- Administer and document skills validation testing for IHSS Attendants before service delivery (or within required regulatory timeframes), and verify ongoing competency in assigned tasks and safety procedures.
- Provide clinical guidance, counseling, and corrective instruction when deficiencies are identified.
- Offer consultation on complex care needs, challenging scenarios, and high-risk situations.
Care Plan Review & Supervision
- Review IHSS Care Plans and Physician Attestation of Consumer Capacity upon enrollment, change in condition, and as required.
- Collaborate with members, authorized representatives, and case managers to determine appropriate levels of clinical oversight; conduct in-home supervision visits as required.
- Validate that Attendants perform tasks in accordance with the approved Care Plan.
Consultation, Documentation & Communication
- Serve as a clinical consultant to members, authorized representatives, IHSS Attendants, and agency staff; collaborate with care teams and support care plan updates when a member's condition changes.
- Maintain accurate, timely documentation of clinical observations, skills validation, supervision visits, care plan reviews, incident investigations, and back-up coverage activities.
- Facilitate effective communication across interdisciplinary teams, including case managers and provider staff.
24-Hour Back-Up Coverage & Direct Care Support
- Participate in the agency's 24-hour back-up system to ensure continuity of IHSS services, responding to urgent staffing gaps, call-offs, or emergencies impacting member safety.
- Provide direct in-home services within RN scope of practice when necessary, and assist with coordinating emergency staffing and contingency planning.
- Support implementation of emergency preparedness plans.
Incident Management & Regulatory Compliance
- Investigate complaints and incidents within required timelines (typically within ten (10) days).
- Ensure compliance with IHSS program regulations, federal and state laws, and agency policies; participate in corrective action planning and risk mitigation.
Training & Quality Assurance
- Assist in developing, delivering, and evaluating Attendant training, including health maintenance activities, infection control, safety procedures, and emergency response.
- Participate in quality assurance and performance improvement initiatives; monitor service delivery trends and identify potential risks.
Required
- Current, unrestricted Registered Nurse (RN) license in the State of Colorado.
- Demonstrated ability to assess clinical competency and supervise unlicensed personnel.
- Strong documentation and communication skills; able to work independently and within a multidisciplinary team.
- Valid driver's license and reliable transportation.
Preferred
- Experience in home health, long-term care, or community-based care
Technical Skills
- Proficiency with smartphones, tablets, or laptops for documentation
- Experience using EMR systems, email, and basic computer applications
Knowledge, Skills & Abilities
- Knowledge of Health Maintenance Activities, delegation principles, infection control practices, and IHSS regulatory requirements and consumer-directed service models.
- Strong clinical assessment, risk identification, and supervision skills; able to respond effectively in urgent clinical or staffing situations.
- Effective interpersonal skills for working with members, families, and care staff.
- Willingness and ability to provide direct care coverage when necessary.
Physical Requirements
- Ability to travel to member homes as required, including frequent driving between client homes.
- Daily standing, walking, bending, squatting, and maneuvering in private homes, including non-barrier-free environments (e.g., tight spaces, stairs).
- Ability to lift over 50 lbs. infrequently (e.g., transfers) and up to 25 lbs. frequently, with frequent use of fine motor skills (grasping, pinching).
- Ability to perform duties associated with clinical observation and in-home supervision.
Work Environment
- Primarily field-based work in member homes, with occasional office responsibilities; homes may be small, cluttered, or not ADA accessible.
- Exposure to variable conditions, including weather, animals, smoke, and unpredictable home environments.
- Participation in on-call rotation or 24-hour back-up coverage system; may require evening, weekend, or holiday availability.
